If you click and hold on the formatting box (it has clear formatting, header 1,
etc.) and then click on the one you want, you get the spinning beachball and
then you have to force quit the app. I have repeated this issue many times. By
the way i have OS 10.4.11.

Cannot confirm this. Tested it with OOO 3.0, 3.0.1 and 3.1 dev build on Mac OS
10.5. 
do you have this problem in a special document or even if you create a new empty
Writer document and apply a style via the mentioned dropdown?
