When the accerciser accesses the hierarchy of accessibility objects of the
Impress tool panel it freezes.

Reason for this is a cyclic parent relationship of accessibility objects.  This
sends the accerciser into a busy loop when it tries to traverse the tree.

Fixed this by creating the accessibility object of the ToolPanelViewShell in its
constructor.
