I try to add the following file to a document and all that I see is a black box.  No pic.
It's just a .jpg file...

I see the black rectangle too, using AOO410 Rev. 1537973.

It is a *.jpg with EXIF information. It starts with FF D8 FF E1.
You can load and save the image by IrfanView to transform it to JFIF kind of *.jpg. It can then be inserted without problems.

Reproduced in Linux, too.
It opens fine in 

3.0.0 OOO300 m9
3.1.1 OOO310 m19 9420
3.2.1 OOO320 m18 9502
3.3.0 OOO330 m20 9567

broken in last Oracle 3.4.0 BETA OOO340 m0 9583 and all Apache versions.

Still confirmed with last 4.1.2.
