When you add buttons to a toolbar and select an icon there are two problems:
1) It displays with a black instead of white background which can make it
illegible (eg icon for insert special character). A restart of OOo sorts this out.
2) If you select an icon when OOo is showing small icons it scales it up
blockily (is there such a word?) when you change icon size to large. When you
restart OOo you get the small icon instead of the matching large one.

Not exactly a show stopper but thought you'd like to know.

could not reproduce with 1.1.1rc3, SuSE 9.0

sorry, don't know how to change icon size to large.. :-(

could not reproduce.
Please tell us 
* How you add the icon (via menu or contextmenu,...)
* What icon you add

Open blank document.
Select small icons if not already small (Tools/Options/OOo/View/Icon size)
Right click on main button bar and choose customize
Choose an entry from the left panel with no icon, say 
Application/Autopilot:fax
click Add to add to the toolbar
Then select the entry you've just added in the right hand panel and click the
icons... button
Select an icon, say the capital T and click OK in the icons panel
You'll notice it's already given a black background.
Click the OK button in the customize dialog  and you'll have a new unreadable
button on the bar.
Changing to Large icons gives an even worse effect!

confirming.
Although I don't get an unreadable icon when I add the icon to the toolbar (not
in the edit-dialog nor on the toolbar itself).
When I switch to large mode, the icons background turns to magenta (OOo's mask
color for transparency). The background stays magenta when I switch to small mode.

Upon restart, the icon is displayed properly again.

When I close OOo while the icons are set to large, a small icon is displayed
instead of the large one (but it is displayed properly).- WHen I switch to small
then, the icon stays small and gets the magenta backgroud.

The attached screenshot shows the following:
top row, left to right:
* Icon was added while toolbar icons were set to small
* Symbol size was set to large
* Symbol size was set to small again

bottom row:
* OOo was restarted while the Icons were set to large

So there definitely is something wrong here, but I assume the black background
depends on the display driver.
