This document can be opened using version 1.1.1, crashes 1.1.2 both Czech and
native English version, there are forms inside the document and is possibly
locked, traceback:

Hi danohnesorg,

thanks for using and supporting OpenOffice.org...

This is just nice to know: both documents open fine on Solaris using OOo1.1.2RC3...

both documents open fine in OOo 1.1.0 (german, self-built) and OOo 1.1.1
(german, official build). OOo 1.1.2 (german, official build) crashes before
displaying anything (on ISDN -specifikace.doc, crash reporter pops up) or hangs
(doesn't eat CPU or MEM, just doesn't react anymore) on the other file.

680m45 crashes on both files (crash reporter comes up)

So I confirm this issue one linux -> crash-keyword, plus I set the newly
introduced regression keyword since OOo 1.1.0 did not have problems opening this
file.

In OCX_Control::Import, there is a line 

uno::Reference<uno::XInterface> xCreate =
		rServiceFactory->createInstance(msFormType);

Unfortunately the string msFormType is empty, which causes an exception to be
thrown in SwXServiceProvider::MakeInstance.

Some further investigations reveal, that the form control actually is of type
WW8FormulaCheckBox, which is derived from WW8FormulaControl, which in turn is a
subclass of OCX_Control. WW8FormulaCheckBox has its own implementation of
::Import, unfortunately this is not called, because the Import function with the
matching signature is _not_ virtual in OCX_Control. Strange. I cannot imagine
that this is intended.

Yes, I have checked this with CWS sw7pp4c, where issue 33980 is fixed and this
one also is fixed with this. But I will not mark this as duplicate, because t
was also used for many other issues as duplicate reference. So I will mark this
as "fixed" for OO 1.1.4, which will be released soon.
